US Passport Renewals

INSTRUCTION GUIDE FOR U.S. PASSPORT RENEWALS
THE FOLLOWING DOCUMENTS ARE REQUIRED:
The Applicant:
o Must submit the most recent expired U.S. Passport
o Must have been 16 years old or older when last U.S. Passport was issued
o Must have a U.S. Passport that was issued within the past 15 years, if passport is beyond 15 years from date of issue you must reapply as a First time applicant.
Please note the following:
o If you lost your valid or recently expired passport, you must follow our instruction guide for "Replacing Lost or Stolen Passports"
o If your expired passport was obtained at age 15 or under, you must follow our instruction guide for "First Time Passport"
o Passports that are damaged or mutilated may not qualify for renewal. You will have to follow our instruction guide for "First Time Passport"
o If your name has changed since you last passport, please include an original or state certified copy of your marriage certificate, divorce decree, or court order showing change of name. Notarized or photocopies are not acceptable
Photographs:
o Two recent original passport size quality photos
o Must be 2"" x 2"" color only
o No black & white photos
Proof of Departure:
o Copy of airline tickets or travel agent itinerary
o Very Important! Without proof of travel you will not be considered an expedite case
o The State Department expedites passports based on travel date or need for travel visas.
Government fees:
o Adults $170.00 payable to DEPARTMENT OF STATE
Letter of Authorization:
o You must complete and include one original letter of authorization signed in Blue Ink

Completed application form DS-82:
o Complete the new Online Version of the DS-82 form,(Complete the form online then click CREATE FORM button) You MUST print and submit ONE of your completed application DS-82
